What Microsatellites Are
Microsatellites are short sequences of DNA, usually made up of 1 to 4 base pairs, that repeat many times in a row. These repeated sequences are found in many places throughout human DNA. The number of repeats can vary from person to person, which makes microsatellites useful for DNA fingerprinting, paternity testing, and forensic identification.
In most cases, microsatellites are located in parts of DNA that do not directly produce proteins, but some are found near or within genes and can affect how genes work. Changes in microsatellites can sometimes lead to diseases or influence cancer behavior.
Why Microsatellites Matter in Cancer
In cancer cells, microsatellites can show instability, meaning the number of repeats changes due to mistakes in DNA copying that the cell cannot fix. This is called microsatellite instability (MSI). When a tumor has a high level of MSI, it is called microsatellite instability-high (MSI-H).
MSI-H cancers often have defects in the cell’s ability to repair DNA errors, which can affect how the cancer grows and responds to treatments. Testing for MSI can help doctors decide if certain treatments, like immunotherapy drugs, might be effective. For example, some drugs are approved to treat advanced cancers with MSI-H or related DNA repair problems.
Where Patients Might Encounter the Term
Patients may see the term microsatellite or microsatellite instability in pathology reports, treatment plans, or when discussing clinical trials. It might be mentioned in relation to cancers such as endometrial cancer, colorectal cancer, melanoma, and others. Sometimes, it appears when discussing specific treatments like immunotherapy drugs that target MSI-H tumors.
Microsatellite tumors can also refer to tiny clusters of cancer cells near the main tumor that can only be seen under a microscope. Finding these can be a sign that the cancer has spread.
What Microsatellite Does Not Automatically Mean
Seeing the word microsatellite or microsatellite instability does not by itself mean a diagnosis or treatment plan. It is a genetic feature of the tumor that helps doctors understand the cancer better. It is important not to assume it defines the entire cancer or the best treatment without discussing it with the care team.
